Hospice Veterans Partnership Discusses VA Benefits, Small Grants: Ohio opportunity deadline March 1

The Hospice Veteran Partnership of Ohio (HVP) held their first conference call of the year on February 5. HVP is a community-based partnership dedicated to increasing veterans' access to hospice and palliative care. Their goals are to strengthen relationships between community hospices and VA providers and educate veterans and their caregivers about end-of-life care options. 

Jill Dunmire Siddiq, Clinical Social Worker-Hospice and Palliative Care with the Cleveland VA Medical Center and Co-Chair of the HVP, offered an overview of veteran eligibility requirements for VA benefits and how to get enrolled.  For more information about how to get a Veteran enrolled for benefits visit or over the phone by calling 1-877-222-VETS (8387).

Emily Marge, Veterans Services Associate with National Hospice and Palliative Care Organization (NHPCO) shared some highlights about the We Honor Veterans program.  Their website will be getting an update in April, making the website easier to use. She extended her congratulations to the six We Honor Veterans Level 5 hospices that we have here in Ohio, who represent the highest level attainable in the We Honor Veterans Program.  To learn how to be a part of We Honor Veterans and what is involved to complete the levels, visit

The 2020 Ohio Small Hospice Provider VFW Rural Hospice Grant application process is now open.  This grant is an opportunity for small hospice providers to apply for funding which is primarily meant to support small hospice programs that provide services to the underserved and/or rural populations, to patients and families without the financial or insurance needs for end-of-life care. Who is eligible, criteria, and how to apply information is available on the LeadingAge Ohio website. If you are interested, the application deadline is March 1, 2020.
